Thank you for taking the time to complete this survey. We’re interested in learning about people’s experiences finding physical activity: what helps, what gets in the way, and what kinds of tools or resources would be useful.

Your answers will help us understand the needs of older adults (50+), caregivers, and healthcare providers so we can develop supports that are easy to use, accessible, and meaningful for the community.

The survey should take about 10-15 minutes, and your responses are anonymous. There are no right or wrong answers. We simply want to hear your perspective. At the end of the survey, you’ll also have the chance to enter a draw for one of four (4) $250 gift cards!*
